Home Care Services
Individual Community Living Support
Caregivers will assist adults who need reminders, cues, intermittent/moderate supervision as well as physical assistance to remain in their homes.
Individualized Home Supports Services
Our caregivers can support, supervise, and train in activities to assist adults in daily living and community activities. We will help build skills, set reminders, and guide adults to achieve these goals.
Medication Management
We will support patients ensuring their use of prescriptions, over-the-counter drugs, vitamins, and supplements are used safely and effectively.
Adult Companion Services
Adult companions meet with people in person or remotely to assist or supervise them with tasks that help the person work toward a therapeutic or community integration goal.
Homemaker Services
A person is eligible to receive homemaker services when either of the following is true:
- The person needs support to manage the general cleaning and household activities.
- The primary caregiver who is regularly responsible for these activities is unable to manage them or is temporarily absent. Refer to the secondary information section for additional information about primary caregivers.
Night Supervision
Our staff will provide overnight assistance and supervision in the patient's own home. Night supervision services include an asleep or awake staff in the person’s own home.
24-Hour Emergency Assistance
On-call counseling, on-call problem solving and/or immediate response for assistance at a person's home due to a health or personal emergency that is not related to an injury or an immediate danger. This service does not replace the use of 911 when necessary.
Respite Care Services
Our caregivers will provide short-term care when their primary caregiver is absent or needs relief. This can be provided in-home or out of home.
